PRE-APPLICATION (PA) Form
In order to begin the membership process, we kindly request that every field of the PRE-APPLICATION (PA) form be filled out; incomplete submissions will result in us being unable to process the PA-Form.
Once the PA-Form is received, we will follow up with a telephone call so that the FINAL APPLICATION (FA) form can be filled out.

FINAL APPLICATION (FA) Form
The CCCK will contact you after the PRE-APLICATION (PA) form is completed and assign your organization a unique I.D. number so that the FINAL APPLICATION (FA) form may be completed.
Applicants are kindly asked to fill out every field of the FA-Form correctly; final membership processing may be delayed if information needs to be verified.

MEMBERSHIP INVOICE
Once the two online PA & FA forms are completed, new members will be emailed CCCK membership invoicing.
Memberships are valid for 12 months, from the month following the date that invoice payment is received; applicants are entitled to attend any of our functions, however, from the time that their membership fee is received.
